Please print and fill out this form if you need MIS to work on any type of project that involves any computer related technology.

Computer Hardware Request Form

Please print and fill out this form if you wish to request any new computer hardware. Submit the form to your supervisor first for their approval.

E-Mail Complaint Form

This form allows you to submit a complaint to the IT Department about Unsolicted E-mail from Leisure World Employees.

New Employee Form

This form helps speed the process for getting User names, setting up e-mail accounts and Passwords.
